SR offers three simple ways for physicians, case managers, or adjustors to refer patients to any of our clinics.

1. You may contact any SR clinic directly - just click on Clinic Locations for the clinic nearest you.  Then either call or fax your referral to the clinic of your choice.
   
   
2. You may contact us via email to refer a patient by clicking here.
   
   Please include the following info within the email:
   
   
   • patient name
   • patient’s date of birth
   • diagnosis or body part being treated
   • date of injury
   • referring physician
   • case manager / adjuster’s name, phone #, fax #
   • claims mailing address
   • # of visits approved
   • employer
   • job description if FCE/PPD rating (allow attachment)
   • claim #
   • patient phone #
   
   
   
3. Our referral process is fully web-enabled. The Express Referral Online System allows referrals to be submitted electronically over our secure server.
